Femoroacetabular impingement (FAI)

Abnormal contact between the hip ball and socket causing pain with deep bending.

Overview

Femoroacetabular impingement occurs when subtle differences in the shape of the hip ball or socket cause them to pinch during deep movements. It can cause groin pain in active people and may involve the labrum. Rehab is the first-line treatment.

Symptoms

  • Groin pain with deep squatting or sitting
  • Pain with twisting or pivoting
  • Stiffness or catching in the hip
  • Reduced hip rotation

Causes

  • Cam or pincer shape variations of the hip
  • Repetitive deep hip loading
  • Sports with pivoting and squatting

Treatment

  • Hip and trunk strengthening
  • Movement and activity modification
  • Pain management
  • Surgical opinion for persistent mechanical symptoms
