Piriformis syndrome

Buttock pain and sometimes leg symptoms attributed to the piriformis muscle.

Overview

Piriformis syndrome describes buttock pain, sometimes with leg symptoms, attributed to the piriformis muscle irritating the nearby sciatic nerve. It is a diagnosis of exclusion and usually responds to strengthening and load management.

Symptoms

  • Deep buttock pain
  • Sometimes leg symptoms mimicking sciatica
  • Pain with prolonged sitting
  • Tenderness deep in the buttock

Causes

  • Overload or tightness of the piriformis
  • Prolonged sitting
  • Hip and trunk weakness

Treatment

  • Hip and gluteal strengthening
  • Load management and sitting modification
  • Nerve mobility where relevant
  • Manual therapy as an adjunct
