SLAP lesion (labral tear)

A tear of the upper shoulder labrum where the biceps tendon attaches.

Overview

A SLAP lesion is a tear of the superior labrum of the shoulder, often involving the biceps anchor. It is common in overhead athletes and can cause deep shoulder pain and catching. Many cases are managed with rehab before considering surgery.

Symptoms

  • Deep shoulder pain, often with overhead activity
  • Catching, clicking or locking sensations
  • Loss of power in throwing or pressing
  • Pain with the arm overhead and rotated

Causes

  • Repetitive overhead throwing
  • A fall onto an outstretched arm
  • Traction injuries to the biceps anchor

Treatment

  • Rotator cuff and scapular strengthening
  • Restoring overhead control
  • Activity and throwing modification
  • Surgical opinion for persistent mechanical symptoms
