Subacromial pain syndrome

Pain at the top and side of the shoulder, often with overhead activity.

Overview

Subacromial pain syndrome (formerly impingement) is the most common cause of shoulder pain. It involves the rotator cuff tendons and bursa in the subacromial space and typically responds well to progressive loading exercise.

Symptoms

  • Pain over the top and outer shoulder
  • Discomfort with overhead and reaching tasks
  • A painful arc of movement
  • Night pain when lying on the shoulder

Causes

  • Rotator cuff overload or weakness
  • Sudden increase in overhead activity
  • Poor scapular control

Treatment

  • Progressive rotator cuff and scapular loading
  • Activity modification
  • Pain management as an adjunct
  • Injection or surgical opinion in persistent cases
